Musical Ethnography of Foster Care in Chicago Media Consent Form:
By submitting your story you’re consenting to have your story become a part of a Musical Ethnography of Foster Care in Chicago. If, at any point, you don’t wish to participate, please contact Sean at (920)810-2701 or [email protected]. To Learn more, read below and/or click here.
What is the purpose of this project?
To increase awareness about foster care through new music
What happens if I choose to participate in this project?
This project will culminate in several performances throughout Chicago in May, 2020 for an audience ranging from 0-150 people. Each section of the performance will explore a different element of foster care. Between each section, an interlude will be played that could feature audio recorded renderings of your story.
What are the possible risks or discomforts?
Sharing your story may trigger memories you don’t wish to remember. If, at any time, you feel triggered we encourage you to reach out to a close friend or loved one to debrief. You may also call the national crisis hotline, open 24/7. Dial 211 or call the national crisis hotline- 1800-273-TALK
What are the possible benefits?
You are not likely to have a direct benefit from participating in this project. However, this project aims to increase awareness of foster care. You will be contacted with the date and time of the project debut, in the event you wish to attend. Your participation is completely voluntary. You will not receive compensation for your time.

Confidentiality will be maintained, as we will be audio recording other people reading your story. If you wish to have an audio recording of yourself telling your story, contact us and we can schedule and in-person or over-the-phone interview, or call 312-487-1873 and leave a short voicemail. By sending this email you are consenting to participate, your story may become a part of MEFCC in May 2020. If, at any time, you wish to not be included, please let us know reach out. We are happy to remove your input from the musical work, no problem!
